To err is human right? Right. But numerous recent articles have demonstrated that a simple checklist can prevent thousands of medical errors, lead to a higher standard of baseline performance and save patient lives. QHN Keep track of your professional development Check on school related materials sent home during the past year. It’s a good idea to keep a separate file on each child. As you review the file, ask your child for his or her comments on school. You might ask, “what do you like best and least?” and “What would you like to change?” The checklist below lists other questions to consider. A list of steps that people can take to make their summer travels more Checklist.... test QHN Responds to Meet the Needs of Ontario Hospitals: Public Reporting on Patient Safety Indicators On May 28, 2008, the Ontario Ministry of Health and Long-Term Care announced that hospitals will be required to publicly report rates on four Safer Healthcare Now! interventions. Hospitals will be required to report rates of MRSA by December 31, 2008 and CLI, VAP, and SSI rates by April 30, 2009. To download Ministry press release click here. In response to this anticipated announcement and to support the work of the campaign in Ontario, QHN is holding a workshop for hospitals preparing to meet the provincial reporting requirements for CLI, VAP and SSI on June 17, 2008 from 7:45 am – 4:15 pm at the Sandman Signature Hotel, Toronto Airport. Ministry to ask hospitals to report on three SHN interventions, Axiom News - June 3, 2008 QHN Staff Member Elected to Greater Toronto Executive of CCHSE It is with great pride and privilege that we announce that Sean Molloy, Director of Knowledge Management for the Quality Healthcare Network and the Ontario Node of Safer Healthcare Now! has been elected to the Greater Toronto Executive of the Canadian College of Health Service Executives (CCHSE). Sean is a recent graduate of the University of Toronto’s HPME program. His interests include knowledge translation, quality improvement, patient safety and effective leadership. His experience in communications, event planning, marketing, and knowledge transfer will allow him to assist the executive in meeting the commitment to providing opportunities for mentorship, engaging colleagues in their early executive careers and in providing leading edge learning opportunities amongst leaders. QHN: Newest Partner in Canada’s Hand Hygiene Campaign The Quality Healthcare Network is pleased to announce that it has partnered with the Canadian Patient Safety Institute (CPSI), Community and Hospital Infection Control Association of Canada, Accreditation Canada, and the Public Health Agency of Canada to support Canada’s Hand Hygiene Campaign. The Canada's Hand Hygiene campaign is aimed at getting organizations ready to adopt and measure hand hygiene practices and compliance within their organizations. This multimodal strategy is based on the World Health Organization’s challenge Clean Care is Safer Care initiative launched worldwide two years ago aimed at raising awareness that healthcare associated infections are an important priority for patient safety. MOH Task Force on Medication Management The Ministry of Health and Long-Term Care recently launched a Task Force on Medication Management in support of their commitment to strengthen medication management practices in the Long-Term Care sector. In partnership with the Ontario Long-Term Care Association (OLTCA), the Ontario Association of Non-Profit Homes & Services for Seniors (OANHSS) and the Institute for Safe Medication Practices Canada (ISMP), they hope to bring together a team of leading experts in the field of medication management in Long-Term Care.
